DONOR ADVISED-FUND
A Donor-Advised Fund (DAF) is a powerful tool that enables you to donate to Health Connected while receiving an immediate tax deduction. With a DAF, you can recommend specific charities or causes to support, and the fund will distribute grants to those organizations on your behalf.
If you cannot use Give Butter or prefer not to use the platform, here are simple steps to donate to Health Connected through your DAF:
- Contact your DAF sponsor and recommend a grant to Health Connected using our Tax ID number (94-3227947).
2. After your DAF sponsor approves the grant recommendation, they will issue a check or send an electronic transfer to Health Connected on your behalf.

Please instruct your DAF sponsor to mail checks to P.O. Box 51984, East Palo Alto, CA 94303.
3. Once we receive your donation, Health Connected will acknowledge your contribution for your records.
